Login to Personal Internet Banking to apply a UOB Supplementary Card.
For Supplementary Card Applicants
Complete your Supplementary Card application online. Simply key in the unique code sent to your email address. The application is to be completed within 10 calendar days from the date you receive the email invitation.

You will be required to provide supporting documents to complete the application.

Eligibility
Supplementary applicants must be at least 18 years of age
(For applicants below 18 years old, please submit a letter of acceptance from the overseas institution.)
Mandatory Documents for Supplementary Card Applicants
1. Singapore Citizen/Permanent Resident
- Copy of NRIC (front and back)
- Latest billing proof (within the last 6 months) as per your local home address (e.g. telephone or utilities bill etc) if it differs from your address in your NRIC
2. Foreigners
- Copy of Passport (with at least 6 months validity and for all nationalities)
- Latest billing proof (within the last 6 months) as per your local home address (e.g. telephone or utilities bill etc)
- Valid Pass Issued in Singapore (e.g. Employment Pass, S-Pass, Work Permit, Entre Pass, Student Pass, Dependent Pass, Long-Term Social Visit Pass)
